2025 AHPN Clinical Care Pathways: Chronic Kidney Disease - Performance Improvement

Chronic Kidney Disease (CKD) affects over 35 million people in the US. This video explains the AdventHealth PHN Clinical Care Pathways process and offers primary care providers dealing with ambulatory chronic diseases an opportunity to improve patient care.

This discussion reviews the guidelines of the National Kidney Foundation and KDIGO (Kidney Disease Improving Global Outcomes) for CKD, providing essential information to ensure their approach to CKD management aligns with the best practices and current standards.

This project fulfills the requirements for PI-CME, ABIM Practice Assessment Part IV and ABFM Performance Improvement, as applicable. While registration identifies your intent to participate, it does not guarantee credits.  Participants must complete all stages and meet the Board Maintenance of Certification Reflection Assessment requirements to earn credits. Awarded credits can include 20 CME credits and 20 ABIM MOC Part IV points or one ABFM PI project and 20 points.

Information for claiming CME and/or Board credits will be sent out end of 1Q26 or early 2Q26.

Certificates are awarded and available to print immediately after the evaluation has been submitted.  If you have a Florida license and would like credit(s) to be uploaded to CE Broker, your license number, including both alpha and numeric data with NO spaces, must be entered on your profile.  CE Broker will not accept credit(s) without the full license number.

You are encouraged to print and save your certificate for your portfolio as required by your licensing body.

At the end of 2025, you will receive notification to complete the Board-required Reflection Assessment indicating completion of all stages. After review and approval, you will be awarded credits.

1. Increase awareness and implement comprehensive screening plans to identify CKD in its early stages.
2. Develop evidence-based guidelines to apply consistent management of CKD at the ambulatory setting.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to:

• 20.00 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program.

• 20.00 Performance Improvement (PI) MOC points in the American Board of Family Medicine Maintenance of Certification (MOC) program.

It is the CME provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
