2025 AHPN Clinical Care Pathways: Type 2 Diabetes Mellitus (IEM)

Diabetes Mellitus 2 (DM2) affects approximately 38.4 million people or 11.6% of the US population. This education will provide healthcare providers the latest guidelines for identifying and direct appropriate treatment of patients with Type 2 Diabetes Mellitus. To ensure the effectiveness of this initiative, 6 DM2 Clinical Care Pathway Performance Monitoring Metrics will be tracked to evaluate the performance of network physicians. This approach provides the difference between current clinical practices with guideline-directed care, ultimately improving patient outcomes and reducing the prevalence of Type 2 diabetes related complications.
