Nearly half of adult Americans suffer from hypertension. The AHPN Hypertension (HTN) Care Pathway aims to educate healthcare providers on the latest guidelines for identifying and treating patients with HTN. To evaluate the performance of network physicians, four HTN Clinical Care Pathway Performance Monitoring Metrics will be tracked.

To indicate your interest in the project:

  1. REGISTER first using the option above.

  2. Select Begin and complete the required Baseline Metrics questions.

  3. Once the baseline is complete, you can watch the video and explore the process materials for additional guidance.

Registration identifies your intent to participate, it does not guarantee credits.  Participants must complete all stages and meet the Board Maintenance of Certification Reflection Assessment requirements to earn credits. This fulfills requirements for the American Board of Internal Medicine (ABIM) MOC Part IV and American Board of Family Medicine  (ABFM) PI projects.

At the end of 1Q26 or early 2Q, participants who have registered and completed the baseline metrics will receive instructions on how to complete the project and claim CME and/or MOC credits.

At the end of 1Q or early 2Q 2026, participants who have registered, completed the baseline metrics will receive instructions on how to complete the required Reflection Assessment indicating completion of all stages. After review and approval, credits will be awarded.



Objectives
At the end of this course, participants should be able to:

  1. Apply the ACC/AHA and ISH guidelines to clinical practice.
  2. Assess patient outcomes and adjust treatment plans based on regular monitoring and reassessment of blood pressure levels.


Non-Physician AMA Attendance

Attendance credit will be provided to other healthcare professionals upon fulfillment of attendance requirements. Participants should ensure in advance their credentialing or licensing organization accepts AMA PRA Category 1 Credit(s)™.
