Heart failure affects over 6.7 million people in the US, costing 30.7 billion dollars annually for treatment. This video explains the AdventHealth PHN Clinical Care Pathways process and offers an opportunity for Primary Care Providers dealing with ambulatory chronic diseases to improve patient care.

This discussion reviews the American Heart Association (AHA) and American College of Cardiology (ACC) recommendations, which includes groups of classes that will be distributed to physicians to help standardize the treatment of heart failure patients according to current AHA/ACC best practice HF guidelines.

There are six HF Clinical Care Pathway Performance Monitoring Metrics that will be monitored to assess the status.

  3. Once the baseline is complete, you can watch the video and explore the process materials for additional guidance.

Registration shows your intent to participate but does not guarantee credits.  To earn CME and/or MOC credits, all stages must be completed, including the Board required Reflection Assessment.

By late Q1 or early Q2 2026, participants who registered and completed the baseline metrics will receive instructions on how to complete the project and claim credits.

Objectives
At the end of this course, participants should be able to:

  1. Review the 2022 American Heart Association/American College of Cardiology 2022 Guideline Directed Medical Therapy standards for managing ambulatory heart failure in the primary care setting.
  2. Discuss the clinical syndrome of heart failure characterized by symptoms and signs resulting from structural or functional impairment or ventricular filling or ejection of blood in the primary care setting.


AdventHealth Orlando is accredited by the Florida Medical Association to provide continuing medical education for physicians.

Designation Statement

AdventHealth Orlando designated this performance improvement activity for a maximum of 20 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

MOC Program Statement

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to:

  • 20 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) Part 2 and 20 ABIM Practice Assessment program; and

  • 20 MOC points in the American Board of Family Medicine's (ABFM) program.

It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM or ABFM MOC credit.
