2025 AHPN Clinical Care Pathways: Chronic Kidney Disease (Video only)

Over 35 million people in the US have Chronic Kidney Disease (CKD). This video explains the AdventHealth PHN Clinical Care Pathways process and provides an opportunity for the PCP dealing with ambulatory chronic diseases to improve patient care. The discussion reviews the guidelines of the National Kidney Foundation and KDIGO (Kidney Disease Improving Global Outcomes) for CKD and provides the PCP essential information ensuring their approach to CKD management is aligned with the best practices and current standards.
