To indicate your interest in the project:

2. Select Begin and complete the required baseline questions.

3. Once the baseline is complete, you can explore the materials for additional guidance.

Registration indicates your intent to participate but does not guarantee CME and/or MOC credits.  Credits will be awarded upon acceptance of the required evaluation, including the Board required Reflection Assessment.  Instructions for completing the evaluation and claiming credit will be provided at the conclusion of the project.

Designation Statement

AdventHealth Orlando designated this performance improvement activity for a maximum of 20 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

MOC Program Statement

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to:

• 20 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) Part 2 and 20 ABIM Practice Assessment program; and

• 20 MOC points in the American Board of Family Medicine's (ABFM) program.

It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM or ABFM MOC credit.
