Radioactive seed localization has improved our rates of negative margins of excision. First, the entry site of the wire is often not at the ideal location for incision by the surgeon at the time of operation. This may lead to additional unnecessary dissection and suboptimal cosmetic results. In addition, the wire must be placed the day of operation, necessitating the coordination of radiology and operative schedules and eliminating the possibility of performing a wire-localized procedure as the first operation of the morning. The most important disadvantage, however, is the inaccuracy of localizing the target lesion transcutaneously and during dissection. The surgeon must estimate the ideal route to the lesion and the amount of tissue to be excised. This results in high rates of reoperations for tissue margins involved by carcinoma.

Objectives
At the end of this course, participants should be able to:

  1. Describe the I-125 Radioactive Seed Localization Program.
  2. Explain how reducing the rates of intraoperative re-excision and re-operation for positive margins is better for the patient.
  3. Recognize that the patient has a five-day window in having the I-125 seed removed versus having the wire placed the day of the operation, necessitating the coordination of radiology and operative schedules.
  4. Eliminate the possibility of performing a wire-localized procedure as the first operation of the morning.
