Heart failure affects over 6.7 million people in the US, costing 30.7 billion dollars annually for treatment. This video explains the AdventHealth PHN Clinical Care Pathways process and offers an opportunity for Primary Care Providers dealing with ambulatory chronic diseases to improve patient care.

This discussion reviews the American Heart Association (AHA) and American College of Cardiology (ACC) recommendations, which includes groups of classes that will be distributed to physicians to help standardize the treatment of heart failure patients according to current AHA/ACC best practice HF guidelines.

There are six HF Clinical Care Pathway Performance Monitoring Metrics that will be monitored to assess the status.

To participate in the Performance Improvement (PI) project, you must REGISTER first and then watch the video by selecting Begin.

This project fulfills the requirements for PI CME, ABIM Practice Assessment Part IV and ABFM Performance Improvement credits, as applicable. While registration identifies your intent to participate, it does not guarantee credits.  Participants must complete all stages and meet the Board Maintenance of Certification Reflection Assessment requirements to earn credits. Awarded credits can include 20 CME credits and 20 ABIM MOC Part IV points or one ABFM PI project and 20 points.

Information for claiming CME and/or Board credits will be sent out at the end of 2025.

At the end of 2025, you will receive notification to complete the Board-required Reflection Assessment indicating completion of all stages. After review and approval, you will be awarded credits.



Objectives
At the end of this course, participants should be able to:

  1. Review ambulatory heart failure management as defined by the American Heart Association/American College of Cardiology 2022 Guideline Directed Medical Therapy standards.
  2. Discuss the heart failure clinical syndrome with symptoms and signs that result from any structural or functional impairment or ventricular filling or ejection of blood.


 

Accreditation and Designation Statements

AdventHealth Orlando is accredited by the Florida Medical Association (FMA) to provide continuing medical education for physicians.
AdventHealth Orlando designates this PI CME activity for a maximum of 20.00 AMA PRA Category 1 Credit(s)™.  Physicians should only claim the credit commensurate with the extent of their participation in the activity.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to:

  • 20.00 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program.

  • 20.00 Performance Improvement (PI) MOC points in the American Board of Family Medicine Maintenance of Certification (MOC) program.

It is the CME provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
